HLPUSD Provision 2 School Sites

Food Services has twenty-seven (27) sites at Hacienda La Puente Unified School District participating in an alternative program call Provision 2. This is available to schools participating in the National School Lunch and School Breakfast Programs for School Year 2020-21. Schools that participate in Provision 2 are able to provide healthy breakfast and lunch each day at no charge for all students enrolled in that school during the 2020-21 School Year. Continuing students do not need to complete any forms at this time. NEW students (such as transitional kindergarten and students transferring from other districts) will need to complete a LCFF Funding Data Collection Form. If you applied for free and reduced price meals for meal benefits, please read below.

Earned Income Tax Credit Information Act Communication

Based on your annual earnings, you may be eligible to receive the Earned Income TaxCredit from the Federal Government (Federal EITC). The Federal EITC is a refundable federal income tax credit for low-income working individuals and families. The Federal EITC has no effect on certain welfare benefits. In most cases, Federal EITC payments will not be used to determine eligibility for Medicaid, Supplemental Security Income, food stamps, low-income housing, or most Temporary Assistance For Needy Families payments. Even if you do not owe federal taxes, you must file a federal tax return to receive the Federal EITC. Be sure to fill out the Federal EITC form in the Federal Income Tax Return Booklet. For information regarding your eligibility to receive the Federal EITC, including information on how to obtain the Internal Revenue Service (IRS) Notice 797 or any other necessary forms and instructions, contact the IRS by calling 1-800-829-3676 or through its website atwww.irs.gov.

 

You may also be eligible to receive the California Earned Income Tax Credit (California EITC) starting with the calendar year 2015 tax year. The California EITC is a refundable state income tax credit for low-income working individuals and families. The California EITC is treated in the same manner as the Federal EITC and generally will not be used to determine eligibility for welfare benefits under California law. To claim the California EITC, even if you do not owe California taxes, you must file a California income tax return and complete and attach the California EITC Form (FTB 3514). For information on the availability of the credit eligibility requirements and how to obtain the necessary California forms and get help filing, contact the Franchise Tax Board at 1-800-852-5711 or through its website at www.ftb.ca.gov.

Food Services accepts meal applications throughout the year. If your financial situation changes, such as reduced wages, you may apply for free or reduced meals online with the Rocket Scan/Online link below or visit the Food Services Office. Dear HLPUSD Community,

Hacienda La Puente Unified School District Food Services Department continues to serve wholesome foods in our school cafeterias. We work closely with food distributor, Gold Star Foods, to provide students with quality meals that are safe to eat. Gold Star has confirmed that foods sourced for school meals are not involved with recent product recalls detailed below. In addition, Food Services only purchases fully cooked proteins for school meals.

On Friday, November 22, 2019, The Center for Disease Control and Prevention (CDC) issued a public health notice. Some romaine lettuce grown in the Salinas growing region could be potentially contaminated with E.coli O157:H7. Food Service supplies are grown in the Yuma region and, thus, are not affected.

On Friday, November 15, 2019, Central Valley Meat Company of Hanford, CA, issued a recall of approximately 34,222 pounds of ground beef products believed to be contaminated with Salmonella Dublin. As Food Services does not purchase this product, we are unaffected.

On Wednesday, November 6, 2019, Simmons Prepared Foods, Inc. issued a recall of 2,071,397 pounds of poultry products that may be contaminated with extraneous materials, specifically metal. Food Services does not purchase from this supplier. We are unaffected.

On Monday, November 4, 2019, King Arthur Flour, Inc. and ADM Milling Company announced a voluntary recall of specific lots of its Unbleached All-purpose Flour (5lb and 25lb) due to a potential presence of E. coli 026 discovered through sampling. As Food Services does not purchase this product, we are unaffected.

On October 31, 2019, North Bay Produce of Traverse City, Michigan announced a voluntary recall of approximately 2,300 cases and two bulk bins of apples. A half dozen apple varieties were recalled due to concerns that the fruit might be contaminated with listeria. The recalled apples were shipped between October 16 and October 21. Food Services purchases apples from CA and WA. We are unaffected.

On September 16, 2019, General Mills announced today a voluntary national recall of five-pound bags of its Gold Medal Unbleached All Purpose Flour with a better if used by date of September 6, 2020. The recall is being issued for the potential presence of E. coli O26 which was discovered during sampling of the five-pound bag product. Food Services does not purchase this product for our meal service. We are unaffected.

On September 6, 2019, Conagra Brand voluntarily recalled approximately 2,200 cases of Udi’s Classic Hamburger Buns due to the potential presence of small pieces of white plastic. Food Services does not purchase this product for our meal service. We are unaffected.

On Thursday, August 31 2019, American Beef Packers, Inc. issued a recall of approximately 24,000 pounds of raw beef products deemed unfit for human consumption. Food Services does not purchase these products. We are unaffected.

On Tuesday, August 20, 2019, the Tip Top Poultry, Inc. issued a recall of 135,810 pounds of fully cooked poultry products that may be mixed with Listeria monocytogenes. Food Services does not purchase this product for our meal service. We are unaffected.

On Thursday, August 15, 2019, the Tyson Foods, Inc. issued a recall of 39,078 pounds of Weaver brand frozen chicken patty products that may be contaminated with extraneous materials. Food Services does not purchase this product for our meal service. We are unaffected.

On July 22, 2019, the Mizkan American, Inc. issued a voluntary recall of potentially contaminated RAGU Pasta Sauce sold at the retail level. The sauce may contain fragments of plastic. Mizkan American, Inc. took this action out of an abundance of caution. Food Services does not purchase this product for our meal service. We are unaffected.

On July 14, 2019, Fieldsource Food Systems, Inc. issued a recall of uncooked, marinated, diced beef and chicken cubes due to misbranding and undeclared allergens. Food Services does not purchase these products for our meal service. We are unaffected.

On June 18, 2019, Nature's Touch Frozen Foods (West) Inc. issued a recall of potentially contaminated Signature Select Avocado Chunks. Food Services does not purchase this product for our meal service. We are unaffected.

On June 14, 2019, Ruiz Foods Product Inc. recalled frozen, not ready to eat breakfast wrap products. Food Services does not purchase these products for our meal service. We are unaffected.

On June 14, 2019, WinCo Foods, LLC recalled frozen red raspberries, 12 ounce bags. The product may be contaminated with norovirus. Food Services does not purchase this product for our meal service. We are unaffected.

On June 11, 2019, Townsend Farms, Inc. recalled domestic conventional frozen blackberry product. The product may be contaminated with Hepatitis A. Food Services does not purchase this product for our meal service. We are unaffected.

On June 13, 2019, King Arthur Flour, Inc. issued a recall of potentially contaminated Unbleached All-Purpose Flour. Food Services does not purchase this product for our meal service. We are unaffected.

On June 7, 2019, Tyson Foods Inc, recalled Tyson Fully Cooked, Whole Grain Golden Crispy Chicken Chunk Fritters. Food Services does purchase this product. We worked with our food distributor, Gold Star Foods, and have confirmed that we are unaffected.

On April 2, 2019, Advance Pierre Foods, Inc. is recalling ready to eat beef patties for the retail market that may be contaminated with extraneous materials, specifically soft purple plastic. Food Services does not purchase this product for our meal service. We are unaffected.

On March 23, 2019, Henry Avocado Corporation issued a voluntary recall of potentially contaminated whole avocados. Food Services does not purchase this product for our meal service. We are unaffected.

On March 21, 2019, Tyson Foods, Inc issued a voluntary recall of potentially contaminated chicken strip products in the retail market. Food Services does not purchase these products for our meal service. We purchase products made for school nutrition that follow USDA guidelines. We are unaffected.

On March 13, 2019, Butterball, LLC issued a voluntary recall of potentially contaminated ground turkey products with Salmonella Schwarzengrund produced on July 7, 2018. Food Services does not purchase raw ground turkey. We are unaffected.

On January 30, 2019, Tyson Foods, Inc. issued a voluntary recall of potentially contaminated Panko Chicken Nuggets. Food Services does not purchase this product. We are unaffected.

On January 29, 2019, General Mills issued a voluntary recall of potentially contaminated five pound bags of its Gold Medal Unbleached Flour with a better if used by date of April 20, 2020. Food Services does not purchase this product. We are unaffected.

US Food and Drug Administration confirmed two separate recalls. Satur Farms, an east coast grower is recalling baby spinach and mesclun. Jac. Vandenburg, from Yonkers, N.Y. and a distributor of fresh fruit products coming from Chile. Items named in the recall were peaches, plums, and nectarines. Gold Star Foods is not connected to either company. Food Services is unaffected.

On December 21, 2018, Jennie-O Turkey Store Sales, Inc. recalled raw ground turkey products that may be contaminated with Salmonella Reading according to the U.S. Department of Agriculture (USDA) Food Safety and Inspection Service (FSIS). Jennie-O Turkey Store Sales, Inc., a Faribault, Minn. establishment, is recalling approximately 164,210 pounds of raw ground turkey products that may be contaminated. These products are available at retail stores. Gold Star Foods confirms they source turkey products prepared for schools only and are unaffected.

On December 17, 2018, the Food and Drug Administration (FDA) published an update on the E. coli outbreak linked to Romaine lettuce reported in November. The Center for Disease Control and Prevention (CDC) and FDA have traced the E. coli strain to a water reservoir in California. On December 13, 2018, Adam Bros. Farming, Inc. in Santa Barbara, CA voluntarily recalled red leaf lettuce, green leaf lettuce, and cauliflower that may have come into contact with water from the contaminated water reservoir. Gold Star Foods has confirmed they and their suppliers do not source product from Adam Bros. Farming, Inc. Produce offered in our HLPUSD cafeterias is wholesome and safe to eat.
