Meal Accommodations for Medical or Special Dietary Reasons

Request for Special Meal Accommodations Due to Medical Condition form:

USDA Regulation 7 CFR Part 15b requires substitutions or modifications in school meals for children with physician documented physical or mental impairment that substantially limits one or more major life activities.  Food & Nutrition Services is committed to provide safe and healthy school meals for students whose impairment restrict their diets. 

We are able to provide the following services for students with identified food allergies:
  • Meal Accommodations as indicated by current physician's information (see form below)
  • Foods to avoid lists
  • Carbohydrate counting tools for students with Diabetes
  • Student Identification when accessing meals (as needed or requested)

The Request for Special Meal Accommodations Due to Medical Condition form explains the impairment and how it affects the student's life activities.  This document and must be signed and dated by a recognized medical authority (a licensed physician, physician assistant, nurse practitioner, or dentist). The Medical Statement includes:

  • An identification of the medical or other special dietary condition which restricts the child’s diet;
  • The food or foods to be omitted from the child’s diet; and 
  • If necessary, the food or choice of foods to be substituted.  (Please note that substitutions provided may be modified based on menus and foods available)
