Application Process

  1. The easiest, fastest and most accurate way to complete an application is online.  We highly recommend this process.
  2. Applications are available at schools, the District Service Center or the Food & Nutrition Services Office. Assure you have the current years application as old applications can not be processed.
  3. Households must list the name of all household members, each household member's income and the source of the income (foster children can be included on the application as a household member).
  4. An adult signature is required along with the last 4 digits of the adult's social security number.  If the signing adult does not have a social security number, it must be indicated on the application.
  5. The information provided on the application may be verified at any time during the school year by District or other program officials.  Auditors of State programs and law enforcement officials may need to access information to look into violations of program rules.
  6. The information may also be shared with education, health, and nutrition programs to help evaluate, fund or determine benefits for their programs.
  7. Boise School District Food & Nutrition Services officials will review the applications and determine eligibility.  Parents or guardians dissatisfied with the ruling of eligibility may discuss the decision with the determining official(s) on an informal basis.  Parents wishing to make a formal appeal may make a request either orally or in writing to Peggy Bodnar, Food & Nutrition Services, 8169 W. Victory Rd., Boise, ID 83709 for a hearing on the decision.
